How care is delivered can be more consequential than the care itself. That idea is central to this week's look at the mechanics of connection on ComplexityPod, where we examine research from different fields. We begin with mobile outreach programs.
This week, we're looking at different aspects of connection—how care is delivered and how understanding is built.
Right, from mobile health outreach to the way we conceptualize a person's entire mental experience. And we even get down to the physics of a liquid droplet on a surface.
Let's start with that mobile outreach. There's research from Sayra M Cristancho on how these programs can help people experiencing homelessness or mental illness, who often face persistent barriers to care.
This work looked at a specific program in Ontario called Supportive Outreach Services. It wasn't just about whether it worked, but how and for whom. They interviewed providers, clients, and system leaders to get a full picture.
And they found that building trust through repeated, non-judgmental contact was a key component for engagement. Also, simply bringing the services to where people are—proximity—was a major factor.
Which makes sense. But the conclusion was that the specific services offered mattered less than the program's ability to reduce friction between people and the system.
So it’s not about just showing up with a list of solutions. It's about building relationships that can adapt across different organizations and sustain themselves over time.
Exactly. That theme of access carries into the next study. Research with Shannon L. Sibbald examines the resources for people with COPD in a region of Canada where the disease is common.
And the scan found quite a few programs—51, all available at no cost.
But that number hides some significant gaps. The study highlighted geographical and infrastructural barriers. Plus, there was only one program tailored specifically for an Indigenous population.
So the services exist on paper, but accessing them is another story. It points to a lack of integration—a system where the pieces don't connect into a smooth pathway for patients.
Pivoting from community health to digital platforms, there's work with member Paul Tremblay that looks at what makes someone stick with a paid mobile health app.
The question was whether you can predict who will pay for a subscription based on how they act during a free trial. This study tracked over 900 new users for a week.
And using that initial data, they identified five different types of users—or behavioral phenotypes. Three of these groups were far more likely to buy a subscription than the others.
So you can spot the likely subscribers early on. But what about keeping them? Did those same groups also renew their subscriptions at higher rates?
No. And that’s a key finding. The analysis showed no significant difference in renewal odds among the groups. The factors that predict an initial purchase don't seem to predict long-term retention.
Which suggests that getting a user to subscribe and getting them to stay are two separate challenges. The early behaviors might get them in the door, but it doesn't guarantee they'll see continued value.
Now, moving from practice to theory, a paper with Lena Palaniyappan tackles a problem in psychiatry: the field has more data than ever, but less coherence.
The argument is that focusing on small, discrete behaviors has fragmented the field's understanding of a person as a whole. It's like having all the puzzle pieces but no image on the box.
To address this, the work proposes reviving Karl Jaspers’ concept of "the whole of psychic life." It aims to make this idea functional for modern research by using five principles: relationality, historicity, subjectivity, intentionality, and indeterminacy.
So it’s a framework to bring the patient's lived experience—their personal story and context—back into a central role, integrating it with biological data instead of being sidelined by it.
And for our final piece, we go from the complexities of the mind to the physics of fluid dynamics. Research with Jerzy M Floryan looks at what happens when a liquid droplet hits a moving surface.
Specifically, what happens to the thin film of gas trapped underneath it. Using high-speed photography, the study watched this film retract into a bubble.
They observed that the two sides of the film behave differently. The upstream point—the one facing the direction of movement—initially moves with the surface, while the downstream point starts by moving in the opposite direction.
So they slip at different rates before eventually catching up to the surface's speed. What factors influence how quickly they sync up?
Two main things: surface tension and viscosity. If you increase either of those properties in the liquid, it takes less time and distance for the contact points to match the surface velocity.
And the researchers were able to build and validate a model that predicts this behavior, based on a balance of forces like surface tension, viscous shear, and inertia.
